Recreational Water Facility Form

Notification to Open or Re-open a Recreational Water Facility

This form must be submitted to us at least 14 days before opening a public recreational water facility that has been closed for more than four weeks or a newly constructed/renovated facility.

An inspection must be conducted by one of our public health inspectors prior to a recreational water facility opening to the public following a closure for more than four weeks, new construction or renovations.

By completing this form, you are fulfilling your legal responsibility to notify Public Health of any opening, re-opening or changes to your recreational water facility.

Owner Information

"Owner" means a person who is the legal or beneficial owner of all or part of the recreational water facility and responsible for ensuring the facility is operated in accordance with O. Reg 565/90.
