Emergencies that require immediate response such as chemical spills, disease outbreak notification, or other major public health incidents should be directed to the staff on call at 1-888-225-7851. 

If you have a medical emergency call 911 or go to the nearest emergency room.

To contact us about your child's immunization record

To update your child’s record or to provide your child’s immunization history to the health unit you can do so in one of the following ways:

• Use the secure on-line form (www.simcoemuskokahealth.org/immsonline)
• Fax a copy of your child’s immunization history to 705-721-1495
• Mail a photocopy of your child’s immunization history to: VPD Program, Simcoe Muskoka District Health Unit, 15 Sperling Drive, Barrie ON, L4M 6K9
• Drop off a photocopy of your child’s immunization history to any one of the health unit office locations
• Call Your Health Connection at 705-721-7520 or 1-877-721-7520 (toll free).
