Parents and guardians of children in Grades 3 and 4 (born in 2016 and 2017) are reminded to make sure their child’s immunization records are up to date by Feb. 3, 2026, to avoid suspension. Under Ontario's Immunization of School Pupils Act (ISPA), students whose immunization records are incomplete or missing can be suspended from school.
In Ontario, there is no provincial-wide digital immunization registry system. When a child receives vaccines from their primary care provider, the details are recorded in their medical record at the doctor’s office. However, this information is not automatically shared with the local health unit. It is the responsibility of parents to report each vaccination to the health unit whenever their child receives a vaccine.
How to Catch Up on Immunizations
- Book an Appointment with Your Health Care Provider
Your family doctor or nurse practitioner can review your child’s immunization record and provide any missing vaccines. - Book a Public Health Clinic Appointment (until January 25)

Check and Update Your Child's’ Immunization Record
You can check your child’s immunization record online at smdhu.org
It’s important to ensure your child’s immunization record is up to date. If your child has received vaccinations outside of the health unit, you must submit this missing information.
