Finding(s):
- Failed to post provincial age restriction sign on entrance door in a visible location (18cmx9cm).
- Failed to post provincial employee reminder signs at point of sale locations (18cmx9cm)
- Failed to post provincial health warning sign near tanning equipment and/or ultraviolet light treatments in a visible location (21.5cmx28cm).
- Failed to post provincial point of sale warning sing near cash register in a visible location (28cmx35.5cm).
Notification of tanning and ultraviolet light businesses must be provided to Simcoe Muskoka District Health Unit (SMDHU)
- Facility failed to notify SMDHU of new tanning and/or ultraviolet light business before offering services to the public.
