Submitting Proof of Vaccination

We only accept and process proof of COVID 19 vaccination for residents of Simcoe County or District of Muskoka. Please contact your local public health unit for directions on submitting proof of vaccination if you are not a resident of Simcoe County or District of Muskoka.

Please:

  • Only submit each proof once. To prevent resubmission please close your browser.
  • Only submit official vaccine receipts from the place where you were vaccinated. If your official vaccine receipt is in a language other than English, please also submit an English translated version.
  • Only jpg, jpeg, or png image files or Adobe PDF are accepted. Microsoft Word files are not accepted.

Submissions are processed in the order of submission dates. Our turn around time is 3-4 business days from date of submission. You will receive an email notification once your proof of vaccine has been entered into the provincial system. Please check your spam or junk folder if you do not see this notification in your inbox.

Proof of Vaccine

For each dose, please upload your proof of vaccine certificate in the matching field. We are not able to process proof of vaccine certificates without a QR code from health authorities where enhanced vaccine certificates with QR codes are available. Visit covid-19.ontario.ca/verify for a list of health authorities issuing enhanced vaccine certificates. If your proof of vaccine does not have your date of birth on it, please submit a piece of identification that has your name and date of birth on it. If your proof of vaccine does not have the name and address of the vaccine clinic where you were vaccinated please provide it in the fields below.
