Update Your Teen's Immunization Record By September 21

 

Immunization Records: Action Required for Grade 11 & 12 Students

Under Ontario’s Immunization of School Pupils Act, all students must have an up-to-date immunization record, a medical exemption, or a signed affidavit on file with the health unit. The health unit is reviewing immunizations records for students in Grade 11 and 12. Students who do not provide the required information by Monday September 21 will be suspended from school starting Tuesday September 22.

How to Catch Up on Missing Immunizations

1. School Immunization Clinics 

We are working with families to help students stay in school and avoid suspensions. Students can attend an immunization clinic at their high school to have their vaccination records reviewed and updated.

Please have your student bring:

  • their yellow immunization card
  • any other proof of their vaccinations, including vaccination dates.

At the clinic, a nurse will:

  • Review your student’s immunization record.
  • Make sure the health unit’s records are up-to-date.
  • Provide any vaccines your student needs.

If you know your student has already received all their vaccines, but their records are not up-to-date with the health unit, please send their yellow immunization card or other vaccination records. Health unit staff can update the records and provide information about how to get vaccination records from your health care provider.

  • Thursday, Sept. 10: St. Dominic Catholic S.S, Huntsville H.S and St. Jospeh’s Catholic H.S
  • Friday, Sept. 11: Orillia S.S. and Bear Creek S.S.
  • Monday, Sept. 14: St. Theresa’s Catholic H.S, Ecole Secondaire Catholique Novelle-Alliance, Elmvale District H.S. and Nantyr Shores S.S.
  • Tuesday, Sept. 15: Stayner Collegiate Institute, Our Lady of the Bay Catholic H.S, Nottawasaga Pines S.S., Ecole Secondaire Romeo-Dallaire, St. Thomas Aquinas Catholic S.S. and Banting Memorial District H.S.
  • Wednesday, Sept. 16: Georgian Bay S.S., Maple Ridge S.S., Innisdale S.S., Bracebridge and Muskoka Lakes S.S., Gravenhurst H.S., Bradford District H.S. and Holy Trinity Catholic H.S.
  • Thursday, Sept. 17: Collingwood Collegiate Institute, Twin Lakes S.S., Patrick Fogerty Catholic S.S., St. Joan of Arc Catholic S.S. and Barrie North Collegiate Institute
  • Friday, Sept. 18: Eastview S.S. and St Peter’s Catholic Secondary School

2. Book an appointment at one of the health unit's clinics Book online at SMDHU 

3. During the week of Sept. 21, attend a drop-in clinic at a health unit office

If you are unable to book an appointment, students can be seen on a first-come, first-served basis at the following times and locations:

  • 15 Sperling Drive Office in Barrie, Collingwood, Cookstown, Midland, Orillia:

 Monday to Friday: 9 a.m. to noon and 1:30 to 4 p.m.

  • Gravenhurst:

Monday to Wednesday and Friday: 9 a.m. to noon and 1:30 to 4 p.m.

  • Huntsville:

Thursday: 9 a.m. to noon and 1:30 to 4 p.m.

Immunization records are automatically updated for students who receive their immunizations through an in-school clinic or at a health unit office.

4. Book an appointment with your health-care provider

Your family doctor or nurse practitioner can review your teen’s immunization record and provide any missing vaccines. Be sure to report the immunizations and dates they were provided to public health as soon as they have been received.

It’s important to ensure your student’s immunization record is up to date. If your teenager receives vaccines from your health-care provider, you must report this immunization information to public health. 

You can easily submit any missing vaccine information:

  • Online: Submit missing information using the ICON portal
  • By Phone: Call 705-721-7520 ext. 8827 or toll-free at 1-877-721-7520

If your family has made the decision not to vaccinate, a parent needs to call the health unit and speak to an immunization nurse to start the affidavit process.
