Report and Request an Immunization Record

How to Report Immunizations

To help keep schools and childcare settings safe and healthy, parents and guardians are responsible for reporting their child's immunizations to their local public health unit.  This is required under Ontario's Immunization of School Pupils Act (ISPA) and the Child Care and Early Years Act (CCEYA).

 In Ontario, health care providers do not automatically report immunization records to public health. Parents and guardians must submit this information directly.

There are two convenient ways to report your child’s immunizations:

  • CANImmunize App - A secure mobile app that allows you to manage and submit immunization records from your smartphone.
  • ICON (Immunization Connect Ontario) Submit records online using a desktop.

Note: there is a delay of up to 2 business days for processing of immunization records submitted electronically (ICON/CANImmunize) due to high volume of submissions. Please submit as soon as possible to avoid suspension.

Request an Immunization Record

For Parents - Accessing a Child’s Immunization Record

Parents and guardians can access their child’s immunization records until the child turns 16 by:

  • Using ICON (Immunization Connect Ontario): A secure online tool that provides a digital alternative to paper records. Access ICON here
  • By Phone: Call 705-721-7520 or 1-877-721-7520, ext. 8827

For Students 16 and Older:

Once a student turns 16, they must request their own immunization record due to privacy regulations.

For Adults – Accessing Your Own Immunization Record

The health unit keeps immunization records for 10 years after your 18th birthday, depending on your birth year and school location:

Born in 1990 or Later:

  • Attended school in Simcoe Muskoka? Your record should be available online through ICON.You’ll need your Ontario health card number to access it.
  • Can’t find your record or don’t have a health card number? Call 705-721-7520 or 1-877-721-7520, ext. 8827 for support.

Born Between 1970 and 1990:

  • Attended school in Simcoe Muskoka?  Your record may be stored in our archive database. Call 705-721-7520 or 1-877-721-7520, ext. 8827 to request a search.

Born Before 1970 or Attended School Outside Simcoe Muskoka:

  • Unfortunately, Simcoe Muskoka District Health Unit (SMDHU) does not have your immunization record on file.

We recommend trying the following:

  • Ask family members if they have copies
  • Contact your childhood health care provider
  • Reach out to the health unit in the region where you attended school
