The Ontario Immunization of School Pupils Act (ISPA), requires the health unit to have up to date immunization records for all students in the Simcoe and Muskoka School Districts.
We need you to:
- Your child’s yellow immunization card
- Your child’s family physician or health-care provider to obtain records, or to get missing vaccinations
- Go to immsonline to enter the complete record, or
- Fax a completed record to 705-726-3962
Sharing your child's immunization record with your health unit is required to ensure everyone is protected.
If you do not provide this information to your health unit, your child could be temporarily suspended from school.*

*Note that for the Meningococcal vaccines, children usually receive their first dose (MEN-C-C) on or after their first birthday. Under the Immunization of School Pupils Act, this information needs to be reported to the health unit before records are reviewed to avoid temporary suspension. A booster dose (MEN-C-ACYW) is required by the end of Grade 7.
