Reporting Your Child's Immunization

If your child receives immunization(s) from your health care provider it is important that you contact the health unit to provide the up to date immunization information. Health care providers do not contact the health unit when they provide immunization to patients.

 

The Simcoe Muskoka District Health Unit keeps immunization records for all elementary and high school students and for children attending day nurseries.  

 

To update your child’s record or to provide your child’s immunization history to the health unit you can do so in one of the following ways:

  • Use the secure on-line form (www.simcoemuskokahealth.org/immsonline)
  • Fax a copy of your child’s immunization history to 705-721-1495
  • Mail a photocopy of your child’s immunization history to: VPD Program, Simcoe Muskoka District Health Unit, 15 Sperling Drive, Barrie ON, L4M 6K9
  • Drop off a photocopy of your child’s immunization history to any one of the health unit office locations
  • Call Your Health Connection at 721-7520 or 1-877-721-7520 (toll free). 

If you are calling the health unit to up date your child’s immunization information or to request a record and receive voice mail please leave the following information:

  • child's first and last name
  • child's date of birth
  • the school or day care attended
  • a telephone number where you can be reached weekdays between 8:30 a.m. and 4:30 p.m.
    Your call will be returned within two business days. 

Immunization records are kept on file for 20 years after the last documented immunization is given. The health unit is able to provide copies of the immunization record to individuals, physicians, other health units and most other places upon the request of the person to whom the record belongs.
