How to Report your child-s vaccinations with the CANImmunize App

Use CANImmunize to enter, manage and submit immunizations from your smartphone!

Download the CANImmunize app on your device.

 

Step 1- Create an account

  • Click “Create an account” if you are not an existing CANImmunize user. If you already have an account, skip to Step 3.
  • Create your account by filling in your name, email, and password. Then click "Sign Up"

 

Step 2 – Verify your account

  • Check your inbox for an email. Click “Verify”, to be redirected to your new CANImmunize account. If you are not redirected, go back to the main screen and login with your email and password.
  • Quick Tip đź’ˇ Check your inbox as well as your junk mail for your verification email

Step 3 – Create your record

  • Click “New Record” to begin entering your vaccines. Once you enter your personal information, you can start adding your vaccination records.
  • Ensure you enter your postal code so you can share the vaccine record with the Simcoe Muskoka District Health Unit.

  

Step 4 – Enter your vaccination information

  • You can add a vaccine by tapping on the age of the “Visit”
  • Select the vaccine(s) received
  • Select the date the vaccine(s) were received and click ok
  • Once done, use the back arrow at the top left to go back. Your entry will automatically be saved
  • If the vaccination received does not appear in the recommended visit, press on the blue Add icon and search for the vaccination that was received.

  Step 5 – Enter your vaccination information & Upload proof of vaccination
  • Using your vaccination record for reference, input the relevant dose information i.e. Date Received, Lot Number etc.
  • You may also upload documents to the app if you desire.
  • Click “Documents” in the Proof of Vaccination section to upload proof of your proof of vaccination.
  • If you received a paper vaccination receipt, upload a photo of the document.
  • If you received a digital vaccination receipt, upload a screenshot of the document as shown to the right

Note: Documents uploaded into the app are NOT shared with the Public Health Unit. They are for your own records only.          

 
 
 
 
 
 
 
 

Step 6 – Ensure you are connected to Simcoe Muskoka District Health Unit

  • If you entered your postal code when you created your profile you may already be connected.
  • Once you have your postal code entered you should see a blue banner. Tap the banner to update/share the record with SMDHU.
  • You can also connect at the bottom of the screen by tapping on the Connect plug

  

  • Once you tap to connect you will need to read the disclaimer and provide consent.
  • Once successful you will get this message
