Update Your Teen's Immunization Record By September 21

 

Immunization Records: Action Required for Grade 11 & 12 Students

Under Ontario’s Immunization of School Pupils Act, all students must have an up-to-date immunization record, a medical exemption, or a signed affidavit on file with the health unit. The health unit is reviewing immunizations records for students in Grade 11 and 12. Students who do not provide the required information by Monday September 21 will be suspended from school starting Tuesday September 22.

How to Catch Up on Missing Immunizations

1. Book an appointment at one of the health unit's clinics - Book online 

2. During the week of Sept. 21, attend a drop-in clinic at a health unit office

If you are unable to book an appointment, students can be seen on a first-come, first-served basis at the following times and locations:

  • 15 Sperling Drive Office in Barrie, Collingwood, Cookstown, Midland, Orillia:

 Monday to Friday: 9 a.m. to noon and 1:30 to 4 p.m.

  • Gravenhurst:

Monday to Wednesday and Friday: 9 a.m. to noon and 1:30 to 4 p.m.

  • Huntsville:

Thursday: 9 a.m. to noon and 1:30 to 4 p.m.

Immunization records are automatically updated for students who receive their immunizations through an in-school clinic or at a health unit office.

3. Book an appointment with your health-care provider

Your family doctor or nurse practitioner can review your teen’s immunization record and provide any missing vaccines. Be sure to report the immunizations and dates they were provided to public health as soon as they have been received.

It’s important to ensure your student’s immunization record is up to date. If your teenager receives vaccines from your health-care provider, you must report this immunization information to public health. 

You can easily submit any missing vaccine information:

  • Online: Submit missing information using the ICON portal
  • By Phone: Call 705-721-7520 ext. 8827 or toll-free at 1-877-721-7520

If your family has made the decision not to vaccinate, a parent needs to call the health unit and speak to an immunization nurse to start the affidavit process.
