Please use the following form to upload documents requested by a member of the Infectious Diseases team related to Diseases of Public Health Significance and/or Outbreak Management.

Forms submitted after 3:00 p.m. will be processed on the next business day. For urgent submissions, please fax the form to 705-733-7738.

NOTE: Due to limitations with the database, when naming files:

Please do not include special characters, for example: ’ ( ) , . / < > etc.
Please keep the length to no more than 50 characters.
File naming suggestions e.g. Line List, Checklist. The facility name and time stamp will automatically be added to the file name when submitted.
