Under Ontario’s Immunization of School Pupils Act, all students must have an up-to-date immunization record, a medical exemption, or a signed affidavit on file with the health unit. The health unit is currently reviewing records for students in Grade 11 & 12. Students who do not provide the required documentation by September 23, 2025 will be suspended from school.
How to Catch Up on Immunizations
- Book an Appointment with Your Health Care Provider
Your family doctor or nurse practitioner can review your teen’s immunization record and provide any missing vaccines. - Book a Public Health Clinic Appointment (until September 12)
You can reserve a spot at a public health immunization clinic while appointments are available: Book online at SMDHU - Attend a Drop-In Clinic (starting September 15)
If you are unable to book an appointment, students can be seen on a first-come, first-served basis at the following times and locations:
Week of September 15
- Monday, Tuesday, Wednesday: 9:00 a.m. – 12:00 p.m. & 1:30 – 4:00 p.m.
- Thursday, Friday: 1:30 – 6:00 p.m.
Week of September 22
- Monday: 1:30 – 6:00 p.m.
- Tuesday through Friday: 9:00 a.m. – 12:00 p.m. & 1:30 – 4:00 p.m.

Check and Update Your Teenager’s Immunization Record
You can check your child’s immunization record online at smdhu.org
It’s important to ensure your child’s immunization record is up to date. If your teenager has received vaccinations outside of the health unit, you must submit this missing information.
