Health Care Connect

HEALTH CARE CONNECT HELPS YOU FIND A FAMILY DOCTOR OR NURSE PRACTITIONER

If you don’t have a family doctor, or your current one is too far away, you can sign up for Health Care Connect:

  • Keep your current doctor
  • Switch only when a closer doctor or nurse practitioner is found

How to sign up

Register with Health Care Connect to be matched with a provider accepting patients in your area:

  • Or call 811 (24 hours a day, 7 days a week)

Who can use it

You must:

  • Have a valid Ontario health card
  • Have an up-to-date mailing address linked to your health card

What you’ll need

  • Name and address
  • Health card number
  • Basic health information

You can sign up for yourself, your family, or someone else (with permission).

What happens next

  • A nurse (care connector) reviews your request
  • They try to find a doctor or nurse practitioner near you
  • You’ll be contacted if a match is found
  • You may be connected to local health services while you wait

Good to know

  • A match may take time
  • People with urgent health needs are prioritized
  • If you decline a match, you’ll need to register again to keep looking
