Register with Health Care Connect to be matched with a provider accepting patients in your area:

Who can use it
You must:
- Have a valid Ontario health card
- Have an up-to-date mailing address linked to your health card
What you’ll need
You can sign up for yourself, your family, or someone else (with permission).
What happens next
- A nurse (care connector) reviews your request
- They try to find a doctor or nurse practitioner near you
- You’ll be contacted if a match is found
- You may be connected to local health services while you wait
Good to know
- People with urgent health needs are prioritized
- If you decline a match, you’ll need to register again to keep looking
