Notification Form For Child Care Centres

New child care centres must provide the Simcoe Muskoka District Health Unit (SMDHU) at least 14 days notice prior to beginning operations to allow time for an inspection to review your file, provide feedback and recommendations regarding requirements and to arrange a date for inspection. Operators should review the new Child Care Centre Physical Requirements and/or Recommendations Checklist.

All child care centres are expected to register with the Ministry of Education prior to registering with public health.
