To help keep all children safe and healthy, licensed childcare programs in Simcoe Muskoka are required by the Child Care and Early Years Act to collect up-to-date immunization information for every child enrolled.

What We Need from You:

• Submit your child’s immunization record using ICON or the CANImmunize Mobile App
• Please also remember to update us any time your child received a new vaccine or booster.

Note: Your health care provider does not automatically report vaccinations to Public Health. It is the parent or caregivers’ responsibility to keep the records up to date with us.

Sharing this information ensures that if there’s ever an outbreak of a vaccine-preventable disease, the health unit can quickly identify children who may be at risk and contact their parents or guardians right away.

For School-Based Child Care:

If your child attends a licensed before- or after-school program that takes place at their school, their immunization records are already collected through the school system — no need to submit them to the childcare program again.

Some families choose not to immunize for religious or personal reasons, or that some children may not be able to receive certain vaccines for medical reasons. If this applies to your family, please get in touch with us at 1-877-721-7520 ext. 8827 so we can help guide you through the steps required to meet the legal exemptions and help keep your child safe in the event of an outbreak.

There are two types of exemptions:

• Medical Exemption: If your child cannot be immunized for medical reasons, a Statement of Medical Exemption must be completed.
• Conscience or Religious Exemption: If you choose not to immunize based on personal beliefs or religion, a Statement of Conscience or Religious Belief is required.
