COVID-19 Assessment Centres and Testing

You can take a self-assessment to help you decide if you need a test. After you’ve been tested, get your results online

You can get a test at a local testing/assessment centre if you:

  • are showing COVID-19 symptoms
  • have been identified as a close contact, and have been informed by public health or by someone in your life who has tested positive or by an exposure notification through the COVID Alert app; 
  • a resident or work in a setting that has a COVID-19 outbreak, as identified and informed by your local public health unit; and 
  • eligible for testing as part of a targeted testing initiative directed by the Ministry of Health or the Ministry of Long-Term Care. 

You do not need a referral to get a test and you do not need a health card, but if you have one please bring it with you. See the Tested - Now what? (revised July 13, 2021) fact sheet for more information. See the locations of local assessment and testing centres below.

You can get tested at a participating pharmacy if you are not showing symptoms, and are eligible for testing as part of a targeted testing initiative as determined by the Ministry of Health or Ministry of Long-term Care or public health.

This includes:

  • residents, workers or visitors of long-term care homes 
  • residents or workers in homeless shelters or other congregate settings 
  • international students that have passed their 14-day quarantine period 
  • farm workers 
  • Indigenous communities

Stevenson Memorial Hospital asks the public to use the Ministry of Health’s COVID-19 self-assessment tool to help determine if they need to come to the assessment centre.

RVH Testing Centre - for booked and drive-thru COVID-19 tests


  • All appointments require your name, health card number and date of birth.
  • The time you are booking is your expected time of arrival.

Southlake Assessment Centre


  • Call 289-803-2199 to make an appointment to be tested at Southlake’s Assessment Centre. 
  • This phone line is open 7 a.m. – 8 p.m. daily.
  • Tip: When you book your appointment, provide your email address. If we have your email address you will be able self-enroll for our Patient Connect portal and get you COVID-19 test results online.

  • Anyone who would like a COVID-19 test must first pre-screen themselves or loved ones to ensure they are eligible for a test.

Residents who do not meet the criteria outlined by the Ministry of Health, will be turned away.

Follow directional signage when you arrive at the Active Living Centre, and our volunteers will review the screening guidelines and arrange your convenient, same day appointment if appropriate.

  • If you are coming to the testing centre please bring your health card and driver’s license. Having both items will help make your time at the centre much quicker.
  • DO NOT enter Rotary Place at the main entrance. Follow the signs and access the testing centre through the dedicated entrance at the side of the building. Please follow the directions for exiting the building. (see map below)
  • Please follow all precautions including wearing a mask, physical distancing and frequent hand hygiene.
  • The time of your appointment is the time that you should arrive at the Assessment Centre. Your test should be completed within approximately 30 minutes of your scheduled arrival time
