Summary of IPAC lapse:

  • Inadequate reprocessing of semi-critical reusable medical equipment/devices prior to each client.
  • Inadequate monitoring and auditing of cleaning and sterilization processes for reusable medical equipment/devices prior to each client.
  • Inadequate administrative controls including policies, staff education and training.

Corrective measures required to address lapse:

  1. Single-use disposable medical equipment/devices to be used until further notice.
  2. Prior to recommencing use of reusable medical equipment/devices ensure reprocessing complies with Provincial Infectious Diseases Advisory Committee (PIDAC) best practices and manufacturers' guidelines.
  3. Policies and procedures, along with staff education and training, should be enhanced for aspects of infection prevention and control including reprocessing of reusable semi-critical medical equipment/devices, monitoring and auditing of instrument reprocessing, and occupational health.
