Summary of IPAC lapse:
- Inadequate reprocessing of semi-critical reusable medical equipment/devices prior to each client.
- Inadequate monitoring and auditing of cleaning and sterilization processes for reusable medical equipment/devices prior to each client.
- Inadequate administrative controls including policies, staff education and training.

Corrective measures required to address lapse:
- Single-use disposable medical equipment/devices to be used until further notice.
- Prior to recommencing use of reusable medical equipment/devices ensure reprocessing complies with Provincial Infectious Diseases Advisory Committee (PIDAC) best practices and manufacturers' guidelines.
- Policies and procedures, along with staff education and training, should be enhanced for aspects of infection prevention and control including reprocessing of reusable semi-critical medical equipment/devices, monitoring and auditing of instrument reprocessing, and occupational health.
