Immunization Records

The health unit only collects immunization information for children and youth currently registered/attending daycare and/or school (elementary and high school only).  Adults are encouraged to keep a copy of their own immunization records.  Please do not use the share button below to submit immunization information for adults.

Immunizations are an important part of your medical records. It is important to keep a copy of your own record as well as your child's.

Our secure online system allows you to:

  • Tell us about the immunizations your child received
  • Get your child’s immunization record*

*Online records are available for birth years 1990 and later. Contact the health unit in the area you attended high school, if you were born before 1990.

The Ontario Immunization of School Pupils Act (ISPA), requires the health unit to have up-to-date immunization records for all students in the Simcoe and Muskoka School Districts.

*At this time, COVID-19 vaccines are NOT required for school entry/attendance. As a result, sharing/reporting of COVID-19 vaccines is not required under ISPA.

We ask you to:

  • Check
    • Your child’s yellow immunization card or health record
  • Contact
    • Your child’s family physician or health-care provider to obtain records, or to get missing vaccinations
  • Share
    • Click the share button above, or
    • Fax a completed record to 705-726-3962
