Proof of vaccination

As of March 1, 2022, proof of vaccination is no longer required to access certain indoor businesses and settings in Ontario.

Why you should register your vaccination

If you get a COVID-19 vaccine outside of Ontario, you should register it so you can:

  • book a second or booster dose (if needed) in Ontario
  • be contacted if there is any clinical guidance about the vaccine you got
  • obtain an enhanced vaccination certificate with QR code that may be required to access certain settings or for travel purposes

In Ontario, an individual is considered fully vaccinated if they have received:

  • A full series of a COVID-19 vaccine authorized by Health Canada , or any combination of such vaccines
  • One or two doses of a COVID-19 vaccine not authorized by Health Canada, followed by one dose of a COVID-19 mRNA vaccine authorized by Health Canada, or
  • Three doses of a COVID-19 vaccine not authorized by Health Canada;

and they received their final dose of the COVID-19 vaccine at least 14 days ago.

Enhanced vaccine certificates with QR code are available:

  • for first and second doses received in Ontario (once you get them)
  • regardless of where you were vaccinated (for example, at a mass immunization clinic, hospital, pharmacy or primary care setting)

Individuals can download their enhanced vaccine certificate. Individuals can also call the Provincial Vaccine Contact Centre at 1-833-943-3900 to have their enhanced vaccine certificate emailed or mailed to them.

To print or download a copy of your enhanced vaccine certificate with QR code, visit https://covid19.ontariohealth.ca. To log in and get your receipt, you will need:

  • a green photo health (OHIP) card (you will need numbers from both the front and back of the card, expired cards will be accepted)
  • your date of birth
  • your postal code (the one associated with your health card)

Individuals who have a red and white health card can call the Provincial Vaccine Contact Centre at 1-833-943-3900 to arrange to receive their vaccine receipt by email.

Individuals who DO NOT have a health card (if you card is lost or expired please call Service Ontario) can call Health Connection 705-721-7520 (1-877-721-7520) to have their identity verified and to receive a COVID ID/Personal Access Code. Individuals can then call the Provincial Vaccine Contact Centre at 1-833-943-3900 to provide this COVID ID in order to arrange to receive their enhanced vaccination certificate by email.

Individuals who do not have an address can call the Provincial Vaccine Contact Centre at 1-833-943-3900 to arrange to receive their enhanced vaccination certificate delivered to a trusted friend, family member or community organization.

We only accept and process proof of COVID 19 vaccination for residents of Simcoe County or District of Muskoka. If you are not a resident of Simcoe County or District of Muskoka you will need to contact your local public health unit for directions on submitting proof of vaccination.

For Simcoe Muskoka residents, please use this secure form to submit your documentation. Submissions are processed in order of submission dates. Our turn-around time is 3-4 business days from date of submission.
