Recreational Camp Manual
Reportable Diseases
Certain diseases with high public health significance are deemed as ‘Reportable’ to the Local Medical Officer of Health under Ontario Regulations 559/91 and amendments of the Health Protection and Promotion Act (HPPA), R.S.O. 1990.
It is recommended that the Simcoe Muskoka District Health Unit be notified immediately if a reportable disease is suspected or confirmed within your recreational camp.
The SMDHU communicable disease team investigators will work with the operator/owner of the camp to effectively manage the reportable illness.
Upon notification you will be asked to provide personal health information which is collected under Section 5 of the HPPA and is used for case management and to provide statistical data to the Ontario Ministry of Health and Long Term Care. The type of information required includes: name of disease being reported, name of person reporting to health unit, name of child/camper/staff member, date of birth, date of onset, name of parent/guardian, home address and any relevant comments or information attributable to the case. For certain respiratory illnesses or outbreaks we may also require your assistance in obtaining relevant immunization history of the camper or staff member to assist in our investigation.
You can choose to contact the health unit directly to provide the above noted information 705 721-7520 (1 877 721-7520) or fax in a completed ‘Notification of Reportable Disease’ form provided by SMDHU to our confidential fax line (number is in hard copy of binder)
