Inadequate cleaning and disinfection of semi-critical equipment and instruments that are reused on clients.

Reuse of equipment and instruments on clients that were designed to e single use and then discarded.

Corrective measures required to address lapse:

1. Equipment/instruments that cannot be cleaned, disinfected or sterilized between each use must be single-use then discarded.

2. Reusable equipment/instruments must be cleaned and disinfected as per the Infection Prevention and Control Best Practices for Personal Services Settings, 2009.

3. Policies and procedures, along with staff education and training, should be enhanced for aspects of infection prevention and control including reprocessing or reusable items and management of accidental exposures to blood or body fluids.
