Receiving a Notice

The Immunization of School Pupils Act states that parents are required to provide the health unit with proof of completed immunization or with the appropriate documentation if they choose not to have their child immunized.  This act also states that all students who do not provide their immunization information to the health unit may be suspended from school until immunization information is received.

 

Immunization questionnaires for round 6 will be mailed home the week of March 12th, 2012

What happens if my child receives an immunization questionnaire or suspension notice?

  • The health unit requires an up to date immunization record prior to suspension day (indicated on the notice)
  • Students whose records are reviewed will initially receive an immunization questionnaire indicating which immunizations are required
  • If the immunization or information is not received by the date requested on the questionnaire, a suspension notice will be issued
  • Prior to mailing of suspension notices, drop in immunization clinics will be held at local health unit offices and are as follows:

*Please note that the offices in Collingwood and Huntsville are closed from 12:30 - 1:30 pm for lunch. 

  • If your student still requires immunization as a result of a suspension notice, it is important that you either book an appointment at your health care provider or contact the health unit at 705-721-7520 or 1-877-721-7520 ext 8807 prior to the suspension date
  • If your child receives immunization at a health care provider you must call the health unit to provide the type of immunization and the date received
