Goal for Intervention: Assist clients to replace one behaviour with another based upon personal experience.
Client Profile Timeline:Client planning to take action in next 4 weeks.
Action (In the process of changing for less than 6 months)
Goal for Intervention: Assist clients to set and meet daily goals .
Client Profile Timeline: Client is in the process of changing or has changed a behaviour in past 6 months.
Maintenance (Has changed for 6 months or longer)
Goal for Intervention: Assist clients to retain the new behaviour.
Client Profile Timeline: Client has maintained change from 6 months to 5 years.
Relapse (Risk behaviour resumed)
Goal for Intervention: Assist clients to see relapse as a normal part of the change process.
Client Profile Timelines: Client has resumed risk behaviour and may be in precontemplation, contemplation or preparation.
