Requirements for Day Nursery Operators

Day Nursery Immunization History Forms:

• Need to be completed for all children enrolled in facility - parents must attach a photocopy of child’s immunization record (yellow card).
• Please send the original form and attached photocopy of the immunization record to the Simcoe Muskoka District Health Unit.  Please be sure to keep a copy at your facility.
• Completed forms should be sent to your contact RN. They can be dropped off at your local health unit office for interoffice mail distribution or mailed to either the Barrie or Gravenhurst office.

  • Day Nursery Immunization History - French 

Class Lists:

• Please mail or fax twice a year in October and February, one complete alpha list
• Include Child’s Name / Child’s DOB / Parent’s Name / Address / Phone #
  • Day Nursery Class List        

Admission/Discharges:

• Complete Day Nursery Admission/Discharge Sheet monthly and fax to health unit
