What is an Adverse Test Result or Adverse Observation?
When something goes wrong with your SDWS, the event may be referred to as either an adverse test result or an adverse observation. These events must be reported to the Health Unit immediately by speaking with a public health inspector.

Regular Business Hours
A public health inspector is available to take reports during regular office hours Monday to Friday 8:30 AM to 4:30 PM at 705-721-7520 or 1-877-721-7520 – Extension 8811.
Important: You must speak directly with a public health inspector. Do not leave a message. If you enter voicemail, reroute to the receptionist and notify them you are calling about an Adverse Result. They will locate an inspector for you.
A public health inspector is available 24/7 to receive reports of adverse test results or adverse observations. If notification is required outside of regular office hours call 1-877-225-7851 to speak with an inspector. After discussing the situation with the public health inspector, written follow-up is required within 24 hours using the Notice of Adverse Test Results and Issue Resolution form. Small drinking water system operators must carefully complete Section 2: Corrective Action and Issue resolution, and fax to the health unit.
